Good 16 months ago. I own Sapphire Nitro R9 390 8GB 1040 MHz model for more than a year. It is a power hungry GPU, but besides that it has excellent cooling, it's more than capable to provide smooth gaming @ 1920x1080 on highest possible settings and also capable pushing every single game out there @ 2560x1440 on high or even highest possible settings. This issue came around today after just over a week with my new PC without a single problem cropping up. To note with the Display Device: I'm using a small TV connected via HDMI cable to the GPU, which I understand might be part of my problem, just to throw the guess out there; I'll be going out to buy a DVI-D/I cable to connect to the TV's VGA input and see if that helps, though I'm still confused as to why it's taken so long for this issue to crop up. (Silly Edit: Realized the GPU has a VGA port on it. So I'll be getting a VGA cable. Because I passed this intelligence roll, thank god.)
At some point with everything working right, either switching between or exiting from a fullscreen game to the desktop view is crashing the display to a black screen. Note that there's no issue with the game itself I can identify before that happens—once this error occurred just viewing the desktop, however, and gave me a green screen instead of black. Upon reboot the Display Adapter, again connected via HDMI cable, is disabled in Device Manager with the nondescript 'Windows has stopped this device because it has reported problems. (Code 43)' error; the audio through the HDMI cable won't work, but I do still get a working screen via Microsoft's basic display adapters.

Uninstalling all (not just the display adapter) AMD Drivers, using the Clean Uninstall Tool, and then downloading and installing the full AMD drivers re-enables the monitor though the display crashes in the midst of the install; a system reboot temporarilly solves all problems and shows all drivers as working correctly, then after some time between 30 minutes to several hours the same problem happens again.
While I'll be getting the DVI-D and/or DVI-I (I'm not sure which one to use, I know only that I've got ports for each on the back of my GPU and a VGA port on the TVs!) tomorrow to see if any problems are fixed, I was wondering if someone with more knowledge might be able to pinpoint the precise problem to help me out. Thanks!
I am having major problems with my card (a SapphireRadeon NITRO R9 380 4GB DL-DVI-I / DL-DVI-D / HDMI / DP Dual-X OC Graphics Card 11242-13-20G)
The card was fine for the first couple of days after I first installed it. It did run very hot and loud but it was running games smoothly. But then when it was taxed it began to show the following strange behavior: the screen would go black for a few seconds and then the screen would come back with the sound from whatever game I was running playing in the background, and the in-game cursor still replacing the regular mouse cursor, but with the game not responding and needing to be closed via task manager. This seems to coincide with memory clock spikes.
I managed to fix this problem by disabling ULPS and increasing voltage to the card but that left me with a lesser version of the problem in which games constantly minimize themselves. The message in the Windows Event Log is as follows:
A request to disable the Desktop Window Manager was made by process (4)
Graphics Card: Sapphire Radeon NITRO R9 380 4GB DL-DVI-I / DL-DVI-D / HDMI / DP Dual-X OC Graphics Card 11242-13-20G
Drivers: Tested every version 15 and 16, currently running 16.5.3
OS: Windows 64 bit home basic
Issue: DirectX games somehow interact badly with the Desktop Window Manager causing constant minimization. OpenGL games, movies, and browsing the internet all function fine, but even non-taxing very old DirectX games cause minimization and seem to cause memory clock spikes. I have tried disabling aero etc. This user has a similar problem [Help] R9 390 driver keeps crashing and now I'm sad : AMDHelp

Leaning towards an RMA right now but I would prefer to avoid the hassle. Any help is appreciated!

Edit: Upon testing some more I have resolved the minimize error in some DirectX games by totally disabling dwm.exe but now some OpenGL games are minimizing without giving any message in the Windows Event logger. Curiouser and curiouser...
